Appointments
Our office hours are by appointment. To schedule an appointment, call between the hours of 8:30 a.m. and 5:00 p.m. Monday through Friday. We close for traditional holidays.

If you need to cancel or reschedule an appointment, notify us as soon as possible.

Telephone
Should you need to call our office with a medical concern, we have a staff of experienced nurses who will assist in answering your questions. If circumstances require a physician consultation, you will receive a response as soon as possible.

After office hours or on days when the office is closed, you can reach the physician on call at 319-234-5764. Our answering service will refer all calls promptly to the physician on call. Please call during office hours for prescription refills, test results or any other non-emergency medical need.

Medical Records & Confidentiality
All your medical records are confidential. We will release your records or any part of them only if you provide us with written authorization.

Fees
Office visit co-payments are due at the time of service. Fees not covered by insurance are due upon notification by the insurance carrier. Our fees are within customary range for this area and reflect the level of care you will receive. If you have any questions regarding our fees, please feel free to discuss them with our Patient Accounts Representative.

Please report any changes in name, address, phone, employment, insurance coverage, etc. in a timely manner.

Insurance
Our office staff will file insurance claims for obstetrical care, surgical care, and P.P.O.s for which we are providers. Please check with your insurance plan to find out if our physicians are providers.

To maximize your insurance benefits, we encourage you to know what your co-pay and insurance benefits are, and make sure we get a copy of your insurance card when services are rendered. Filing insurance claims is a service provided without charge and in no way relieves your responsibility for your bill.
